Information you choose to provide

The consultation form asks for your name, email address, and message. If you use the form after a Health Check in the same browser, it may also include an optional reference to that Health Check follow-up.

Contact and Health Check submissions

A contact message is recorded and may trigger an operational notification to Tomlinson Glover Legacy Group LLC when notification settings are available.

The Health Check asks for contact and company context, including contact name, email, company, employee count, industry, work states, current HR support, an optional immediate issue, and answers to its questions. The app records those responses and creates a score, summary, and suggested next steps. A result or confirmation email may be sent to the address provided, and some submissions may also trigger an operational notification.

Paid assessment information

If you pursue a paid assessment, information and results from your Health Check or assessment may be associated with that assessment and considered for the review included in its agreed scope. The specific materials and whether any human review takes place depend on the applicable service or matter. Any human review is limited to the agreed scope; this page does not guarantee that a particular item will be personally reviewed.

Conditional site analytics

If a deployment enables analytics by setting its analytics slug, the browser creates or reuses a polsia_vid value in local storage and sends a page-load beacon to the configured Polsia analytics endpoint.
